I, the undersigned parent, legal guardian, or participant (if 18 years of age or older), acknowledge that participation in Journey League Basketball activities involves physical activity and the potential risk of injury. I understand that these risks may include, but are not limited to, minor injuries, serious injuries, or other unforeseen risks associated with participation in athletic activities. I knowingly and voluntarily assume all such risks on behalf of myself and/or the participant. In consideration of being allowed to participate in Journey League Basketball programs, events, practices, games, and related activities, I hereby release, waive, discharge, and hold harmless Journey League Basketball League, its officers, directors, employees, volunteers, coaches, sponsors, partners, and affiliates from any and all claims, demands, actions, or causes of action arising out of or related to any injury, illness, damage, or loss sustained during participation, except in cases of gross negligence or willful misconduct. I further acknowledge that Journey League Basketball is a volunteer-run organization and that reasonable accommodations will be made to support participants with special needs; however, I understand that not all risks can be eliminated. I certify that the participant is physically and medically able to participate in league activities, or that appropriate medical guidance has been provided. I agree to assume full responsibility for any medical expenses incurred as a result of participation. I have read and fully understand this Hold Harmless and Release of Liability and sign it freely and voluntarily.
